Proof of Permanent address

If applicant is staying out of the state for the last one year, he/she should mention both the addresses prescribed in the online application. Besides furnish the permanent address proof with Aadhar card, Voter ID , Gas connection or Bank Account pass book with entry page for last six months.

Government Employee

Proof of Date of Birth

Date of birth certificate issued by any recognized board.
